PPO/Out of Network Plans
Many PPO plans have out-of-network benefits that you may elect to use for providers outside of your network. Often these benefits offer even greater flexibility than your in-network benefits (i.e. do not have session limits, greater restriction of your health record, etc.) If you would like to use your PPO benefits for my services, please contact me to discuss how it would work. I am happy to help with any forms or questions to complete this process and typically can submit claims directly to your insurance.
To find out whether your health insurance plan will cover a portion of our services, please contact them directly and ask the following:
1) Does my plan cover out-of-network mental health services (CPT codes to verify are 90834/90837 -- 90837 often offers more coverage)?
2) They will usually give you a percentage of fee covered (i.e., 70% of a "reasonable and customary fee"), so you will need to ask them what their fee typically is for the CPT code you have given them. Then we can do the math and see what amount they will be reimbursing you.
3) Is there preauthorization required or special circumstance to use CPT code 90837?

Cash, check, debit or credit cards (including HSA & FSA cards) accepted.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 48 hours in advance, you will be required to pay the full cost of the session. The only exception made will be in the event of a significant illness or emergency.
